bullet ADULT DAY SERVICES

Nurse Manager –F/T (Hauppauge)
Maintain highest quality professionalism in nursing services. Confer with agency medical staff and other local, state, and federal authorities regarding treatments, confidentiality, and laws. Responsible for scheduling, organization, assignments, training, implementation and oversight of any and all Nursing services. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. Minimum five years related experience and/or training. Supervisory/Management experience required. NYS Nursing License and valid NYS driver’s license, available vehicle.

Day Hab Assistants– F/T & P/T (Hauppauge)
Responsible for the implementation of individualized services in all Day Habilitation areas emphasizing community integration and in-house activities to enhance the People Served in their chosen outcomes affecting his/her capacity to live, work, socialize, and participate within the community. High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience. Valid clean New York State Driver's License required and Valid First Aide Certification (Responsibility of employee).

Teacher Aide F/T & Subs (Commack)
Assists classroom teacher in curriculum implementation by working with individuals or groups of children as assigned by classroom teacher. Assists in implementing strategies suggested by clinicians to the teacher. Performs frequent daily lifting of individuals with disabilities (ages 2-21), for their activities of daily living needs. High school diploma or general education degree (GED) required.

Residence Managers (Various locations)
Manages one subordinate supervisor (Assistant Residence Manager) who supervises a total of up to 13 employees in the Residence. Responsible for the overall direction, coordination, and evaluation of these units. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. High school diploma or general education degree (GED) and one to three years related experience and/or training. Supervisory experience in the field of developmental disabilities necessary. Valid clean NYS Driver's License required. SCIP-R, CPR, and AMAP certification preferred.

Assistant Residence Managers (Various locations)
Assist Residence Manager to perform all duties related to oversight of assigned residential program serving individuals with developmental disabilities. Function as Manager in Manager's absence. Supervises a total of up to 13 employees in the Residence. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. High school diploma or general education degree (GED) and one to six months related experience and/or training. Supervisory experience in the field of developmental disabilities necessary. Valid clean NYS Driver's License required. SCIP-R, CPR, and AMAP certification preferred.

Program Supervisor (Various Locations)
Responsible for overall administration of assigned residences to include; Regulation Compliance, staff supervision & training, monitoring of budgets and physical plant issues, and over all Ensure quality of care for the individuals served. Must have knowledge and experience in the operation of multiple sites, staff training and incident management. Knowledge of OPWDD regulations in ICF/IRA. On-call position. Bachelor’s degree and 3+ years experience required.

Case Manager (Commack)
Act as a primary advocate for assigned group of clients, coordinates and supervises Interdisciplinary Treatment Team. Primary liaison with day programs and clinicians. Minimum requirement of Bachelor’s degree, Master’s degree preferred. Must be eligible for QIDP status. Experience with the developmentally delayed and disabled preferred. Valid clean NYS Driver's License required throughout employment. Completion of OPWDD Medication Administration (AMAP) course, Strategies in Crisis Intervention and Prevention (SCIP) course, First Aid, Driver’s Training and Cardiopulmonary Resuscitation (CPR) and Residential Performance Standards Certification within the probationary period.

Cook (Per Diem) Commack
Provide nutritious meals, box lunches and snacks in regulation with the special dietary needs of the participants. High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.

Direct Support Professionals- F/T, P/T & Per Diem (Various Locations)
Assist individuals with developmental disabilities in activities of daily living. High school degree or GED required. Must have a clean and valid NYS Driver’s License.

FACILITIES

Custodian (F/T Hauppauge)
Maintains building, performing minor and routine painting, plumbing, and other related maintenance activities. Insure that facility is free of unsafe conditions and conform to agency standards, state and federal regulations. High school diploma or general education degree (GED) and one to three months related experience and/or training. Must have a minimum one-year experience in housekeeping or similar field and valid current New York State Driver License.
