bullet ADULT DAY SERVICES

Habilitation Specialist - Temp (Marcus)
As per OPWDD regulation (690), acts as primary QIDP (Qualified Intellectual Disabilities Professional) for participants within his/her program room. This includes supervision of the implementation of the person’s program plan, insuring the integration of the various treatment plan services received by the person, recording the person’s progress, assuring that all documentation is current and accounted for and leading periodic reviews of the person’s program plans within the caseload. Provides appropriate individualized instruction to meet the needs of program participants. Responsible for protective oversight and safeguards of the individuals assigned throughout the program day. Bachelor's degree (B. A.) from four-year college or university in a field of education, social work, psychology, training rehabilitation or a related human services field from an accredited program or permanent certification from the NYS Department of Education in the field of education. Program Aide (Marcus)
Assists in implementation and recording of program services for individuals as outlined in each individual’s plan of care. Responsible for the protective oversight and safeguards of the individuals assigned throughout the program day. High school diploma or general education degree (GED). SCIP-R, AMAP and CPR certification preferred.

Independent Living Specialist (Marcus)
Responsible for the development and implementation of individualized programs for individuals served in all Day Habilitation services areas emphasizing community integration and in-house activities to enhance the People Served chosen goals effecting his/her capacity to live, work, socialize, and participate within the community. Bachelor’s Degree in related field and experience with working with people with Developmental Disabilities or five years experience working directly with persons with Developmental Disabilities in capacity required by essential job duties. Valid New York State Driver's License. Valid First Aid Certification.

Physical Therapist (Commack) 12 week leave replacement F/T
Responsible for the evaluation and treatment of those students who have been referred by CSE/CPSE and physician script has been obtained, and assigned by the Clinical Supervisor. Bachelor's degree (B. A.) from four-year college or university. Graduate of a school accredited by APTA. Two years "pediatric/rehabilitation" experience preferred. NYS License or eligibility upon graduation necessary.

Teacher Subs (Commack)
Responsible for the education of each student on a day-to-day basis. Follow daily routine as per the classroom teacher’s plan book. Complete program documentation in accordance with school/department requirements. Assist in performing frequent daily lifting of individuals with disabilities (ages 2-21) for their activities of daily living needs. Directly supervises teacher aides. Previous teaching experience with DD population is preferred.

Teacher Aides LTT & Subs (Commack)
Assists classroom teacher in curriculum implementation by working with individuals or groups of children as assigned by classroom teacher. Assists in implementing strategies suggested by clinicians to the teacher. Performs frequent daily lifting of individuals with disabilities (ages 2-21), for their activities of daily living needs. High school diploma or general education degree (GED) required.

Residence Managers (Various locations)
Manages one subordinate supervisor (Assistant Residence Manager) who supervises a total of up to 13 employees in the Residence. Responsible for the overall direction, coordination, and evaluation of these units. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. High school diploma or general education degree (GED) and one to three years related experience and/or training. Supervisory experience in the field of developmental disabilities necessary. Valid clean NYS Driver's License required. SCIP-R, CPR, and AMAP certification preferred.

Assistant Residence Managers (Various locations)
Assist Residence Manager to perform all duties related to oversight of assigned residential program serving individuals with developmental disabilities. Function as Manager in Manager's absence. Supervises a total of up to 13 employees in the Residence. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. High school diploma or general education degree (GED) and one to six months related experience and/or training. Supervisory experience in the field of developmental disabilities necessary. Valid clean NYS Driver's License required. SCIP-R, CPR, and AMAP certification preferred.

Case Manager (Various locations)
Act as a primary advocate for assigned group of clients, coordinates and supervises Interdisciplinary Treatment Team. Primary liaison with day programs and clinicians. Minimum requirement of Bachelor’s degree, Master’s degree preferred. Must be eligible for QIDP status. Experience with the developmentally delayed and disabled preferred. Valid clean NYS Driver's License required throughout employment. Completion of OPWDD Medication Administration (AMAP) course, Strategies in Crisis Intervention and Prevention (SCIP) course, First Aid, Driver’s Training and Cardiopulmonary Resuscitation (CPR) and Residential Performance Standards Certification within the probationary period.

Food Service Supervisor (F/T) Commack
Schedule and supervise all kitchen staff in the performance of their job duties. Flex work schedule to ensure thorough orientation of new cooks, meal observations, and in-servicing of residential staff. Cooperate with the dietitian in menu development and modification with emphasis on variety, quality, and seasonal changes. .Food purchasing/inventory control utilizing both vendors and local markets to ensure adequate quality, quantity, freshness and variety. .Ensure that the kitchen and all food storage areas meet high sanitary standards. Responsible for good maintenance of all kitchen equipment and supplies. Regularly conduct and document food service inspections (at least twice monthly). High school diploma or general education degree (GED) and five to ten years related experience and/or training.

Cook (Per Diem) Commack
Provide nutritious meals, box lunches and snacks in regulation with the special dietary needs of the participants. High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.

Registered Nurse F/T (Commack) Tues – Saturday 3pm -11:30 pm
Responsible for monitoring and assisting in the overall health and safety of the individuals served .Insure that all indicated medical appointments are scheduled and followed up appropriately Maintenance of all medical records per state and federal guidelines. Attend annual/interim meetings, weekly IDT meetings, and staff meetings. Participate in supervision and in-service training of direct care staff. Meet regularly with residence managers to discuss current health care concerns (minimum once weekly). Associate's degree (A. A.) or equivalent from two-year college or technical school and one to six months' related experience and/or training; or equivalent combination of education and experience. New York State - Licensed Registered Nurse required.

Direct Support Professionals- F/T, P/T & Per Diem (Various Locations)
Assist individuals with developmental disabilities in activities of daily living. High school degree or GED required. Must have a clean and valid NYS Driver’s License.

FACILITIES

Fleet Maintenance/ Project Supervisor F/T (Hauppauge)
Directs and coordinates the service and repair of all agency vehicles, coordinates with vendors and Residential staff on specific renovations/remodeling projects and assists the Maintenance staff with routine daily maintenance. The ideal candidate must possess excellent computer skills, have basic mechanical knowledge and skills, and have a good general knowledge of vehicle maintenance and repairs HS Diploma or GED and valid and clean NYS Drivers License required.

Custodian F/T (Hauppauge)
Maintains building, performing minor and routine painting, plumbing, and other related maintenance activities. Insure that facility is free of unsafe conditions and conform to agency standards, state and federal regulations. High school diploma or general education degree (GED) and one to three months related experience and/or training. Must have a minimum one-year experience in housekeeping or similar field and valid current New York State Driver License.

FINANCE

Staff Accountant (Marcus)
Prepares/inputs journal entries with appropriate supporting documentation. Maintain numerous state grants and their expenditures. Compares budgets to actual expenses and explains variances. Completes year end account analysis for audit. Completes Year End schedules for state reports. Maintains Fixed Assets schedules. Completes monthly fringe analysis schedule as well as misc schedules needed for management. Bachelors degree in Accounting required. Strong Excel Skills a must. 3-5 years experience necessary.
