ADULT DAY PROGRAMS
Program Supervisors (2) Hauppauge
Assist with the day-to-day administration and implementation of all operational and Day Habilitation or Day Treatment program requirements. This is a hands-on management position. Bachelor's degree (B. A.) from four-year college or university in a field of education, social work, psychology, training rehabilitation or a related human services field from an accredited program or permanent certification from the NYS Department of Education in the field of education. Credentials must meet qualifications for OPWDD QMRP. Must be computer literate: word & excel SCIP-R and AMAP certification preferred.
Program Aides - Per Diem (Hauppauge)
Assists in implementation and recording of program services for individuals as outlined in each individual’s plan of care. Responsible for the protective oversight and safeguards of the individuals assigned throughout the program day. High school diploma or general education degree (GED) required. SCIP-R, AMAP and CPR certification preferred.
ADULT DAY SERVICES
Physical Therapist F/T (Commack)
Responsible for the evaluation and treatment of those students who have been referred by CSE/CPSE and physician script has been obtained, and assigned by the Clinical Supervisor. Directly supervises PT Assistants, limited permits, and students. Responsibilities include training/in-servicing employees in work related functions. Bachelor's degree (B. A.) from four-year college or university. Graduate of a school accredited by APTA. Two years pediatric/rehabilitation experience preferred.
Teacher Aides F/T and Subs (Commack)
Assists classroom teacher in curriculum implementation by working with individuals or groups of children as assigned by classroom teacher. Assists in implementing strategies suggested by clinicians to the teacher. Performs frequent daily lifting of individuals with disabilities (ages 2-21), for their activities of daily living needs. High School Diploma or general education degree (GED) required.
Assistant Director of Human Resources (Hauppauge)
Responsible for assisting in the planning, organizing, and carrying out of policies relating to all phases of HR activities by performing some or all of the HR duties personally or through other HR team members. Will also serve as the "Acting" Director in the Director's absence. Bachelor's degree (B. A.) from four-year college or university; or Master' degree in Human Resources Management required. Five (5) plus years of Human Resources Generalist experience with three (3) years of experience in a management role necessary. Senior Professional in Human Resources (SPHR) or Professional in Human Resources (PHR) certification preferred.
Residence Managers (Various locations)
Manages one subordinate supervisor (Assistant Residence Manager) who supervises a total of up to 13 employees in the Residence. Responsible for the overall direction, coordination, and evaluation of these units. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. High school diploma or general education degree (GED) and one to three years related experience and/or training. Supervisory experience in the field of developmental disabilities necessary. Valid clean NYS Driver's License required. SCIP-R, CPR, and AMAP certification preferred.
Assistant Residence Managers (Various locations)
Assist Residence Manager to perform all duties related to oversight of assigned residential program serving individuals with developmental disabilities. Function as Manager in Manager's absence. Supervises a total of up to 13 employees in the Residence. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. High school diploma or general education degree (GED) and one to six months related experience and/or training. Supervisory experience in the field of developmental disabilities necessary. Valid clean NYS Driver's License required. SCIP-R, CPR, and AMAP certification preferred.
Nursing Supervisor (F/T) Various Locations
To oversee and coordinate all medical and nursing service delivery for all programs. Responsible for all administrative functions related to supervision of nursing staff. Supervises employees in residential nursing. Associate's degree (A. A.) or equivalent from two-year college or technical school and three to five years related experience and/or training. NYS nursing license. Supervisory experience.
Registered Nurses (P/T & F/T Eves)
Responsible for monitoring and assisting in the overall health and safety of the individuals served. Insure that all indicated medical appointments are scheduled and followed up appropriately. Insure maintenance of all medical records per state and federal guidelines. Attend annual/interim meetings, weekly IDT meetings, and staff meetings. Participate in supervision and in-service training of direct care staff. Meet regularly with residence managers to discuss current health care concerns (minimum once weekly). Associate's degree (A. A.) or equivalent from two-year college or technical school and one to six months' related experience and/or training; or equivalent combination of education and experience. New York State - Licensed Registered Nurse required.
Cook Housekeeper F/T (East Northport)
Provide nutritious meals, box lunches and snacks in regulation with the special dietary needs of the participants. Insure cleanliness and proper sanitation of ICF. This includes resident’s bedrooms, dining room, nursing station, corridors, lobby, lounge, recreation room and laundry room. High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.
Direct Support Professionals- F/T, P/T & Per Diem (Various Locations)
Assist individuals with developmental disabilities in activities of daily living. High school degree or GED required. Must have a clean and valid NYS Driver’s License.
Physical Therapist F/T (Central Islip)
Responsible for the evaluation of patients referred for Physical therapy services and the formulation of a treatment care plan based on best current practices. Treatment of patients through the use of physical agents, appropriate therapeutic exercise, stretching, joint mobilization or other therapeutic technique and re-evaluation of patients on program. Bachelor's degree (B. A.) from four-year college or university and one to three years of experience (OMRDD population preferred). New York State License in Physical Therapy NYS Licensed Physical Therapist required.
Manager of Facilities (Hauppauge)
HANDS-ON individual who will be responsible for facilities services. Manager will be responsible for all building and property maintenance, repairs, cleaning, fleet maintenance, landscaping, snow removal, and the day-to-day facilities requirements of agency programs. Coordinate the preventative maintenance of all facilities equipment and building systems. Functions as Safety Officer for all commercial facilities. Manages and maintains department staff schedules. Monitor and evaluate all facilities staff or contracted vendor’s work to assure that it meets established quality standards. Coordinate and oversee contracted services (landscaping, snow removal, plumbing, electrical, HVAC, etc.) including development of specifications, biding, and contracting. Ideal candidate must have at least 5 – 8 years facilities management experience with excellent communication, interpersonal, project management, computer and leadership skills. Associates Degree required.
Custodian – P/T (Commack)
Maintains building, performing minor and routine painting, plumbing, and other related maintenance activities. Insure that facility is free of unsafe conditions and conform to agency standards, state and federal regulations. High school diploma or general education degree (GED) and one to three months related experience and/or training. Must have a minimum one-year's experience in housekeeping or similar field and valid current New York State Driver License.
