ADULT DAY PROGRAMS
Program Supervisor F/T (Hauppauge)
As per OPWDD regulation (690), acts as primary QMRP (Qualified Mental Retardation Professional) for participants for assignments. This includes supervision of the implementation of the person’s program plan, insuring the integration of the various treatment plan services received by the person, recording the person’s progress and leading participant review meetings for the assigned caseload. Provides supervision and appropriate individualized instruction to Program Aides to meet the needs of program participants. Assures timely and accurate data documentation for the purpose of the compliance with state and federal regulations. Bachelor's degree (B. A.) from four-year college or university in a field of education, social work, psychology, training rehabilitation or a related human services field from an accredited program or permanent certification from the NYS Department of Education in the field of education. Credentials must meet qualifications for OPWDD QMRP. Must be computer literate: word & excel. SCIP-R and AMAP certification preferred.
Program Aides – Per Diem (Hauppauge)
Assists in implementation and recording of program services for individuals as outlined in each individual’s plan of care. Responsible for the protective oversight and safeguards of the individuals assigned throughout the program day. High school diploma or general education degree (GED) required. SCIP-R, AMAP and CPR certification preferred.
Community Habilitation Asst. Managers P/T (Hauppauge)
Responsible for the implementation of individualized services in all Community Habilitation areas emphasizing community integration and in-home activities to enhance the individuals served in their chosen outcomes to live, work, volunteer, socialize and participate within their community to their maximum independence. High school diploma or Test Assessing Secondary Completion (TASC); One year related experience and/or training. Current valid NYS Drivers license. Current First Aide/CPR Certification (within probationary period). Others as required by regulation changes.
ADULT DAY SERVICES
Service Coordinator F/T – (Hauppauge)
Provides information, linkage, referral, and monitoring of services; advocacy to empower individuals and families; and help to advance the independence, productivity, and full citizenship of people with developmental disabilities. Minimum two years' service coordination/case management exact experience and Bachelors Degree meeting the QIDP- Qualified Intellectual Disability Professional requirements. To perform this job successfully, an individual should have knowledge of Spreadsheet software and Word Processing software. Clean valid NYS Driver's license and a reliable vehicle required.
Employment Specialist F/T (Hauppauge)
Community Employment services including job development, assessments, intensive skills and all related employment training,( especially with computers) and follow along services necessary to maintain community employment for facility and non-facility based participants with disabilities. Minimum, Bachelor's degree (B. A.) from four-year college or university in related field and one to three years related experience or (5 years' exact job development experience and/or certification in career/employment can be substituted). Master's preferred. Experience in business/industry preferred and/or vocational rehabilitation of adults. Valid clean NYS Driver's License. Current First Aid Certification or to obtain within 3 months from the date of hire.
Teacher of the Visually Impaired P/T (Commack)
Provides appropriate individualized instruction to meet the needs of students according to their Individualized Family Service Plans and Individualized Education Plan.Minimum, Bachelor's degree (B. A.), and a Master of Science degree from a four-year college or university; some experience preferable. NYS Certification as a Teacher of the Blind/Partially Sighted required.
Physical Therapist - F/T (Commack)
Responsible for the evaluation and treatment of those students who have been referred by CSE/CPSE and physician script has been obtained, and assigned by the Clinical Supervisor. Directly supervises PT Assistants, limited permits, and students. Responsibilities include training/in-servicing employees in work related functions. Bachelor's degree (B. A.) from four-year college or university. Graduate of a school accredited by APTA. Two years pediatric/rehabilitation experience preferred.
Clerk/Typist- F/T (Commack)
Responsible for the general department filing, typing of memos and forms, computer data entry (Word Processing knowledge), office phones, receptionist substituting when necessary. Disperses mail, and tracks reports. To perform this job successfully, an individual should have knowledge of Contact Management systems; Spreadsheet software and Word Processing software. Ability to type 40 wpm. High school diploma or general education degree (GED) and some related experience and/or training.
Registered Nurses- F/T & Per Diem (Commack)
Provide first aid and emergency treatment for the students. Administer medications and treatments as prescribed. Provide tuberculin testing. Minimum of two-year college or graduate school of nursing and one to six months related experience and/or training. A current NYS license to practice as a registered nurse is required.
Teacher Aides Subs (Commack)
Assists classroom teacher in curriculum implementation by working with individuals or groups of children as assigned by classroom teacher. Assists in implementing strategies suggested by clinicians to the teacher. Performs frequent daily lifting of individuals with disabilities (ages 2-21), for their activities of daily living needs. High School Diploma or general education degree (GED) required.
Residence Managers (Various locations)
Manages one subordinate supervisor (Assistant Residence Manager) who supervises a total of up to 13 employees in the Residence. Responsible for the overall direction, coordination, and evaluation of these units. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. High school diploma or general education degree (GED) and one to three years related experience and/or training. Supervisory experience in the field of developmental disabilities necessary. Valid clean NYS Driver's License required. SCIP-R, CPR, and AMAP certification preferred.
Assistant Residence Managers (Various locations)
Assist Residence Manager to perform all duties related to oversight of assigned residential program serving individuals with developmental disabilities. Function as Manager in Manager's absence. Supervises a total of up to 13 employees in the Residence. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. High school diploma or general education degree (GED) and one to six months related experience and/or training. Supervisory experience in the field of developmental disabilities necessary. Valid clean NYS Driver's License required. SCIP-R, CPR, and AMAP certification preferred.
Nursing Supervisor - (F/T) Various Locations
To oversee and coordinate all medical and nursing service delivery for all programs. Responsible for all administrative functions related to supervision of nursing staff. Supervises employees in residential nursing. Associate's degree (A. A.) or equivalent from two-year college or technical school and three to five years related experience and/or training. NYS nursing license. Supervisory experience.
Registered Nurses - (P/T) Various Locations
Responsible for monitoring and assisting in the overall health and safety of the individuals served. Insure that all indicated medical appointments are scheduled and followed up appropriately. Insure maintenance of all medical records per state and federal guidelines. Attend annual/interim meetings, weekly IDT meetings, and staff meetings. Participate in supervision and in-service training of direct care staff. Meet regularly with residence managers to discuss current health care concerns (minimum once weekly). Associate's degree (A. A.) or equivalent from two-year college or technical school and one to six months' related experience and/or training; or equivalent combination of education and experience. New York State - Licensed Registered Nurse required.
Licensed Practical Nurse - (P/T) Various Locations
Under the supervision of a Registered Nurse, assist in the overall health and safety of the individuals served. Administer medications and treatments as prescribed by physician. Generate incident, seizure, and illness reports. Monitor and report health care on daily flow report records. Instruct mid-level supervisors and direct care staff in above, i.e., hygiene, diet, weights, etc. Submit written monthly, quarterly, annual nursing/medical progress notes. Associate's degree (A. A.) or equivalent from two-year college or technical school and one to six months' related experience and/or training; or equivalent combination of education and experience. New York State Licensed Practical Nurse.
Behavior Intervention Specialist - F/T (Various locations)
Develop, maintain goal plans related specifically towards addressing behaviors of individuals served under to supervision of a licensed Psychologist. Responsible for evaluation and testing individuals served and training of staff members. Master's degree (M. A.) or equivalent; or four to ten years related experience and/or training; or equivalent combination of education and experience. M.A. degree in Applied Psychology, from an accredited training program (including internship) e.g., school, clinical, counseling, and educational.
Adult Services Assistant - F/T (Commack)
Perform administrative duties under the direction of the Nursing Supervisor. Administrative duties include scheduling appointments, maintaining medical records, billing, and coding for insurance purposes. High school diploma or general education degree (GED) and one to six months related experience and/or training. Prior experience in a medically related field a plus. To perform this job successfully, an individual should have basic to intermediate knowledge of Microsoft Office software (Word, Outlook, Excel).
Direct Support Professionals - F/T, P/T & Per Diem (Various Locations)
Assist individuals with developmental disabilities in activities of daily living. High school degree or GED required. Must have a clean and valid NYS Driver’s License.
