bullet ADULT DAY SERVICES

Habilitation Specialist - Temp (Marcus)
As per OPWDD regulation (690), acts as primary QIDP (Qualified Intellectual Disabilities Professional) for participants within his/her program room. This includes supervision of the implementation of the person’s program plan, insuring the integration of the various treatment plan services received by the person, recording the person’s progress, assuring that all documentation is current and accounted for and leading periodic reviews of the person’s program plans within the caseload. Provides appropriate individualized instruction to meet the needs of program participants. Responsible for protective oversight and safeguards of the individuals assigned throughout the program day. Bachelor's degree (B. A.) from four-year college or university in a field of education, social work, psychology, training rehabilitation or a related human services field from an accredited program or permanent certification from the NYS Department of Education in the field of education. Program Aide (Marcus)
Assists in implementation and recording of program services for individuals as outlined in each individual’s plan of care. Responsible for the protective oversight and safeguards of the individuals assigned throughout the program day. High school diploma or general education degree (GED). SCIP-R, AMAP and CPR certification preferred.

Teacher Aide F/T & Subs (Commack)
Assists classroom teacher in curriculum implementation by working with individuals or groups of children as assigned by classroom teacher. Assists in implementing strategies suggested by clinicians to the teacher. Performs frequent daily lifting of individuals with disabilities (ages 2-21), for their activities of daily living needs. High school diploma or general education degree (GED) required.

Residence Managers (Various locations)
Manages one subordinate supervisor (Assistant Residence Manager) who supervises a total of up to 13 employees in the Residence. Responsible for the overall direction, coordination, and evaluation of these units. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. High school diploma or general education degree (GED) and one to three years related experience and/or training. Supervisory experience in the field of developmental disabilities necessary. Valid clean NYS Driver's License required. SCIP-R, CPR, and AMAP certification preferred.

Assistant Residence Managers (Various locations)
Assist Residence Manager to perform all duties related to oversight of assigned residential program serving individuals with developmental disabilities. Function as Manager in Manager's absence. Supervises a total of up to 13 employees in the Residence. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. High school diploma or general education degree (GED) and one to six months related experience and/or training. Supervisory experience in the field of developmental disabilities necessary. Valid clean NYS Driver's License required. SCIP-R, CPR, and AMAP certification preferred.

Case Manager
Act as a primary advocate for assigned group of clients, coordinates and supervises Interdisciplinary Treatment Team. Primary liaison with day programs and clinicians. Minimum requirement of Bachelor’s degree, Master’s degree preferred. Must be eligible for QIDP status. Experience with the developmentally delayed and disabled preferred. Valid clean NYS Driver's License required throughout employment. Completion of OPWDD Medication Administration (AMAP) course, Strategies in Crisis Intervention and Prevention (SCIP) course, First Aid, Driver’s Training and Cardiopulmonary Resuscitation (CPR) and Residential Performance Standards Certification within the probationary period.

Behavior Intervention Specialist (Various locations)
Develop, maintain goal plans related specifically towards addressing behaviors of individuals served under to supervision of a licensed Psychologist. Responsible for evaluation and testing individuals served and training of staff members. Master's degree (M. A.) or equivalent; or four to ten years related experience and/or training; or equivalent combination of education and experience. M.A. degree in Applied Psychology, from an accredited training program (including internship) e.g., school, clinical, counseling, and educational.

Cook (Per Diem) Commack
Provide nutritious meals, box lunches and snacks in regulation with the special dietary needs of the participants. High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.

Direct Support Professionals- F/T, P/T & Per Diem (Various Locations)
Assist individuals with developmental disabilities in activities of daily living. High school degree or GED required. Must have a clean and valid NYS Driver’s License.

FACILITIES

Custodian F/T (Hauppauge)
Maintains building, performing minor and routine painting, plumbing, and other related maintenance activities. Insure that facility is free of unsafe conditions and conform to agency standards, state and federal regulations. High school diploma or general education degree (GED) and one to three months related experience and/or training. Must have a minimum one-year experience in housekeeping or similar field and valid current New York State Driver License.
