bulletADULT DAY PROGRAMS

Habilitation Specialist – Temp (Marcus)
As per OPWDD regulation (690), acts as primary QIDP (Qualified Intellectual Disabilities Professional) for participants within his/her program room. This includes supervision of the implementation of the person’s program plan, insuring the integration of the various treatment plan services received by the person, recording the person’s progress, assuring that all documentation is current and accounted for and leading periodic reviews of the person’s program plans within the caseload. Provides appropriate individualized instruction to meet the needs of program participants. Responsible for protective oversight and safeguards of the individuals assigned throughout the program day. Bachelor's degree (B. A.) from four-year college or university in a field of education, social work, psychology, training rehabilitation or a related human services field from an accredited program or permanent certification from the NYS Department of Education in the field of education. Day Habilitation Assistants F/T (Marcus)
Responsible for the implementation of individualized services in all Day Habilitation areas emphasizing community integration and in-house activities to enhance the People Served in their chosen outcomes affecting his/her capacity to live, work, socialize, and participate within the community. High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience. Valid clean New York State Driver's License required and Valid First Aide Certification (Responsibility of employee).

Independent Living Specialist (Marcus)
Responsible for the development and implementation of individualized programs for individuals served in all Day Habilitation services areas emphasizing community integration and in-house activities to enhance the People Served chosen goals effecting his/her capacity to live, work, socialize, and participate within the community. Bachelor’s Degree in related field and experience with working with people with Developmental Disabilities or five years experience working directly with persons with Developmental Disabilities in capacity required by essential job duties. Valid New York State Driver's License.

Program Aides F/T, Temp and Per Diem (Marcus)
Assists in implementation and recording of program services for individuals as outlined in each individual’s plan of care. Responsible for the protective oversight and safeguards of the individuals assigned throughout the program day. High school diploma or general education degree (GED). SCIP-R, AMAP and CPR certification preferred.

bullet ADULT DAY SERVICES

Coordinator of Employment Services (Marcus)
Management position to coordinate all aspects of employments services and development and implementation among individuals with disabilities, employment staff and community. Build and maintain business/community contacts and partnerships. Work as part of the Vocational Rehabilitation Management team. Bachelors Degree req’d, Masters preferred and 5+ years experience in management of NYS employment options and implementation for individuals with disabilities. Supervisory experience necessary. Excellent communication and computer skills (Excel, ACCESS, Project Mgmt., Contact Mgmt). Current First Aid and Valid and Clean NYS Drivers license required with access to own vehicle.

Teacher Aides Subs (Commack)
Assists classroom teacher in curriculum implementation by working with individuals or groups of children as assigned by classroom teacher. Assists in implementing strategies suggested by clinicians to the teacher. Performs frequent daily lifting of individuals with disabilities (ages 2-21), for their activities of daily living needs. High School Diploma or general education degree (GED) required.

Residence Managers (Various locations)
Manages one subordinate supervisor (Assistant Residence Manager) who supervises a total of up to 13 employees in the Residence. Responsible for the overall direction, coordination, and evaluation of these units. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. High school diploma or general education degree (GED) and one to three years related experience and/or training. Supervisory experience in the field of developmental disabilities necessary. Valid clean NYS Driver's License required. SCIP-R, CPR, and AMAP certification preferred.

Assistant Residence Managers (Various locations)
Assist Residence Manager to perform all duties related to oversight of assigned residential program serving individuals with developmental disabilities. Function as Manager in Manager's absence. Supervises a total of up to 13 employees in the Residence. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. High school diploma or general education degree (GED) and one to six months related experience and/or training. Supervisory experience in the field of developmental disabilities necessary. Valid clean NYS Driver's License required. SCIP-R, CPR, and AMAP certification preferred.

Case Manager (Various locations)
Act as a primary advocate for assigned group of clients, coordinates and supervises Interdisciplinary Treatment Team. Primary liaison with day programs and clinicians. Minimum requirement of Bachelor’s degree, Master’s degree preferred. Must be eligible for QIDP status. Experience with the developmentally delayed and disabled preferred. Valid clean NYS Driver's License required throughout employment. Completion of OPWDD Medication Administration (AMAP) course, Strategies in Crisis Intervention and Prevention (SCIP) course, First Aid, Driver’s Training and Cardiopulmonary Resuscitation (CPR) and Residential Performance Standards Certification within the probationary period.

Registered Nurse (F/T) Commack
Responsible for monitoring and assisting in the overall health and safety of the individuals served. Insure that all indicated medical appointments are scheduled and followed up appropriately. Insure maintenance of all medical records per state and federal guidelines. Attend annual/interim meetings, weekly IDT meetings, and staff meetings. Participate in supervision and in-service training of direct care staff. Meet regularly with residence managers to discuss current health care concerns (minimum once weekly). Associate's degree (A. A.) or equivalent from two-year college or technical school and one to six months' related experience and/or training; or equivalent combination of education and experience. New York State - Licensed Registered Nurse required.

Direct Support Professionals- F/T, P/T & Per Diem (Various Locations)
Assist individuals with developmental disabilities in activities of daily living. High school degree or GED required. Must have a clean and valid NYS Driver’s License.
