bulletADULT DAY PROGRAMS

Day Habilitation Asst. - F/T (Bohemia)
Responsible for the implementation of individualized services in all Day Habilitation areas emphasizing community integration and in-house activities to enhance the People Served in their chosen outcomes affecting his/her capacity to live, work, socialize, and participate within the community. High school diploma or general education degree (GED); or one to three months related experience and/or training. Valid New York State Driver's License required.

Program Aides – Per Diem (Hauppauge)
Assists in implementation and recording of program services for individuals as outlined in each individual’s plan of care. Responsible for the protective oversight and safeguards of the individuals assigned throughout the program day. High school diploma or general education degree (GED) required. SCIP-R, AMAP and CPR certification preferred.

Service Coordinator - F/T (Hauppauge)
Provides information, linkage, referral, and monitoring of services; advocacy to empower individuals and families; and help to advance the independence, productivity, and full citizenship of people with developmental disabilities Minimum two years service coordination/case management exact experience and Bachelors Degree meeting the QIDP- Qualified Intellectual Disability Professional requirements. OPWDD Core Service Coordinator Training within 6 months of hire. Interpersonal skills, organization & planning, problem solving, conflict resolution ability, assertiveness, strong teamwork & networking skills, ability to carry out job duties independently in the community. Clean valid NYS Driver's license and a reliable vehicle.

Physical Therapist - F/T (Commack)
Responsible for the evaluation and treatment of those students who have been referred by CSE/CPSE and physician script has been obtained, and assigned by the Clinical Supervisor. Directly supervises PT Assistants, limited permits, and students. Responsibilities include training/in-servicing employees in work related functions. Bachelor's degree (B. A.) from four-year college or university. Graduate of a school accredited by APTA. Two years pediatric/rehabilitation experience preferred.

Clerk/Typist- F/T (Commack)
Responsible for the general department filing, typing of memos and forms, computer data entry (Word Processing knowledge), office phones, receptionist substituting when necessary. Disperses mail, and tracks reports. To perform this job successfully, an individual should have knowledge of Contact Management systems; Spreadsheet software and Word Processing software. Ability to type 40 wpm. High school diploma or general education degree (GED) and some related experience and/or training.

Registered Nurses- Per Diem (Commack)
Provide first aid and emergency treatment for the students. Administer medications and treatments as prescribed. Provide tuberculin testing. Minimum of two-year college or graduate school of nursing and one to six months related experience and/or training. A current NYS license to practice as a registered nurse is required.

Teacher Aides Subs (Commack)
Assists classroom teacher in curriculum implementation by working with individuals or groups of children as assigned by classroom teacher. Assists in implementing strategies suggested by clinicians to the teacher. Performs frequent daily lifting of individuals with disabilities (ages 2-21), for their activities of daily living needs. High School Diploma or general education degree (GED) required.

Residence Managers (Various locations)
Manages one subordinate supervisor (Assistant Residence Manager) who supervises a total of up to 13 employees in the Residence. Responsible for the overall direction, coordination, and evaluation of these units. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. High school diploma or general education degree (GED) and one to three years related experience and/or training. Supervisory experience in the field of developmental disabilities necessary. Valid clean NYS Driver's License required. SCIP-R, CPR, and AMAP certification preferred.

Assistant Residence Managers (Various locations)
Assist Residence Manager to perform all duties related to oversight of assigned residential program serving individuals with developmental disabilities. Function as Manager in Manager's absence. Supervises a total of up to 13 employees in the Residence. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. High school diploma or general education degree (GED) and one to six months related experience and/or training. Supervisory experience in the field of developmental disabilities necessary. Valid clean NYS Driver's License required. SCIP-R, CPR, and AMAP certification preferred.

Nursing Supervisor - (F/T) Various Locations
To oversee and coordinate all medical and nursing service delivery for all programs. Responsible for all administrative functions related to supervision of nursing staff. Supervises employees in residential nursing. Associate's degree (A. A.) or equivalent from two-year college or technical school and three to five years related experience and/or training. NYS nursing license. Supervisory experience.

Registered Nurses - (P/T & F/T)
Responsible for monitoring and assisting in the overall health and safety of the individuals served. Insure that all indicated medical appointments are scheduled and followed up appropriately. Insure maintenance of all medical records per state and federal guidelines. Attend annual/interim meetings, weekly IDT meetings, and staff meetings. Participate in supervision and in-service training of direct care staff. Meet regularly with residence managers to discuss current health care concerns (minimum once weekly). Associate's degree (A. A.) or equivalent from two-year college or technical school and one to six months' related experience and/or training; or equivalent combination of education and experience. New York State - Licensed Registered Nurse required.

Licensed Practical Nurse - F/T & P/T
Under the supervision of a Registered Nurse, assist in the overall health and safety of the individuals served. Administer medications and treatments as prescribed by physician. Generate incident, seizure, and illness reports. Monitor and report health care on daily flow report records. Instruct mid-level supervisors and direct care staff in above, i.e., hygiene, diet, weights, etc. Submit written monthly, quarterly, annual nursing/medical progress notes. Associate's degree (A. A.) or equivalent from two-year college or technical school and one to six months' related experience and/or training; or equivalent combination of education and experience. New York State Licensed Practical Nurse.

Direct Support Professionals - F/T, P/T & Per Diem (Various Locations)
Assist individuals with developmental disabilities in activities of daily living. High school degree or GED required. Must have a clean and valid NYS Driver’s License.

Assistant Director of Human Resources (Hauppauge)
The ideal candidate will have five plus years of progressive human resources generalist experience with at least three years' supervisory experience, preferably in a not-for-profit or healthcare environment. Must have both strategic and “hands-on" experience with recruiting, compensation, benefits administration and compliance, training and development, the ability to partner with program management to investigate and resolve employee and labor relations issues and compliance. Responsibilities will also include the oversight of the day-to-day operations of the HR department. Individual must have the ability to work well under pressure and handle multiple priorities in a fast paced environment. Bachelor's degree (B. A.) from four-year college or university; or Master' degree in Human Resources Management. Five (5) plus years of Human Resources Generalist experience with three (3) years of experience in a management role. Senior Professional in Human Resources (SPHR) or Professional in Human Resources (PHR) certification preferred.

DEVELOPMENT

Assistant Director of Development for Special Events (Hauppauge)
Oversee signature fundraising events for UCP Suffolk and ensure the organization of producing profitable income through special events. Create and coordinate comprehensive event fund-raising plans to adequately fund agency’s current and long-term financial needs. Establish and maintain recognized, respected UCP presence in the community. Maximize all existing fundraising campaigns and develops new contacts and opportunities. Bachelor's degree (B. A.) from four-year college or university; or five to ten years related experience and/or training.

FACILTIES

Maintenance Mechanic - (Various Locations)
Maintain each facility in a clean, comfortable and safe environment for participants and staff. Perform manual work in building repair and grounds care. Perform assigned minor alteration work in facilities. Assemble, repair and move furniture. Inspect assigned facilities on a monthly basis, making certain agency standards, state and federal regulations. Valid current New York State Driver License required. HS Diploma or GED necessary.
