bullet ADULT DAY SERVICES

Service Coordinator (P/T) Hauppauge
Provides information, linkage, referral, and monitoring of services; advocacy to empower individuals and families; and help to advance the independence, productivity, and full citizenship of people with developmental disabilities. Minimum two years service coordination/case management exact experience and Bachelors Degree meeting the QIDP- Qualified Intellectual Disability Professional requirements. Clean valid NYS Driver's license and a reliable vehicle required.

Program Aides – F/T (Hauppauge)
Assist Habilitation Specialist in developing, organizing, implementing and assessing programs for individuals. High school diploma or general education degree (GED).

Day Hab Assistant – F/T (Hauppauge)
Responsible for the implementation of individualized services in all Day Habilitation areas emphasizing community integration and in-house activities to enhance the People Served in their chosen outcomes affecting his/her capacity to live, work, socialize, and participate within the community. High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience. Valid clean New York State Driver's License required and Valid First Aide Certification (Responsibility of employee).

Habilitation Specialist Supervisor - Long-term temp (Bohemia)
Management position for supervision of the Independent Living Specialists, Program assistants, Aides, bus driver. High school diploma or general education degree (GED); Five to ten years in related field and working directly with persons with Developmental Disabilities in capacity required by essential job duties To perform this job successfully, an individual should have basic to intermediate knowledge of Microsoft Office software (Word, Outlook, Excel). Valid New York State Driver's License and Valid First Aide Certification (responsibility of employee).

Occupational Therapist P/T – 7.5 Hours per week (Commack)
Responsible for the evaluation and treatment of those consumers who have been referred by CSE/CPSE and physician script has been obtained, and assigned by the Clinical Supervisor. Bachelor's degree (B. A.) and two years related experience necessary. NYS License/Certification or eligibility upon graduation- Certification by the American Occupational Therapy Association required..

Clerk/Typist F/T (Commack)
Responsible for the day-to-day typing, filing, computer input, and any other clerical duties needed by the department of whom they are assigned. High school diploma or general education degree (GED) and some related experience and/or training. To perform this job successfully, an individual should have knowledge of Contact Management systems; Spreadsheet software and Word Processing software. Ability to type 40 wpm necessary.

Teacher Aide Subs (Commack)
Assists classroom teacher in curriculum implementation by working with individuals or groups of children as assigned by classroom teacher. Assists in implementing strategies suggested by clinicians to the teacher. Performs frequent daily lifting of individuals with disabilities (ages 2-21), for their activities of daily living needs. High school diploma or general education degree (GED) required.

Coordinator of Residential Support Services (Commack)
Responsible for organization, presentation, and documentation of the Residential Program’s orientation and training programs, including development and review of curriculum/training objectives, coordination of presenters, ensuring sufficient materials, attendance, room arrangements, etc. Bachelor's degree (B. A.) from four-year college or university or comparable hands-on experience with physically challenged or developmentally disabled.

Residential Coordinator (Commack)
Assist Program Supervisor in the coordination and delivery of services needed to insure health and well being of program participants. Bachelor's degree (B. A.) from four-year college or university or comparable hands-on experience with physically challenged or developmentally disabled.

Residence Managers (Various locations)
Manages one subordinate supervisor (Assistant Residence Manager) who supervises a total of up to 13 employees in the Residence. Responsible for the overall direction, coordination, and evaluation of these units. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. High school diploma or general education degree (GED) and one to three years related experience and/or training. Supervisory experience in the field of developmental disabilities necessary. Valid clean NYS Driver's License required. SCIP-R, CPR, and AMAP certification preferred.

Assistant Residence Managers (Various locations)
Assist Residence Manager to perform all duties related to oversight of assigned residential program serving individuals with developmental disabilities. Function as Manager in Manager's absence. Supervises a total of up to 13 employees in the Residence. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. High school diploma or general education degree (GED) and one to six months related experience and/or training. Supervisory experience in the field of developmental disabilities necessary. Valid clean NYS Driver's License required. SCIP-R, CPR, and AMAP certification preferred.

Developmental Specialist F/T (Commack)
Plan, evaluate and structure ADL programs which will be incorporated into each daily activity schedule, including the individual’s goals, methodology and objectives that will be developed, monitored and updated as needed. Bachelor's degree in Special Education from four-year college or university and one to three years related experience and/or training.

Direct Support Professionals- F/T, P/T & Per Diem (Various Locations)
Assist individuals with developmental disabilities in activities of daily living. High school degree or GED required. Must have a clean and valid NYS Driver’s License.

Intake Coordinator – 11:30a-7:30p (Central Islip)
Assist with admissions, pre-certification, doctor appointments and inquiries. To perform this job successfully, an individual should have basic to intermediate knowledge of Microsoft Office software (Word, Outlook, Excel). High school diploma or general education degree (GED) required. Candidates must be bi-lingual (Spanish speaking).
