ADULT DAY PROGRAMS
Day Habilitation Assistants - F/T (Hauppauge & Bohemia)
Responsible for the implementation of individualized services in all Day Habilitation areas emphasizing community integration and in-house activities to enhance the People Served in their chosen outcomes affecting his/her capacity to live, work, socialize, and participate within the community. High school diploma or general education degree (GED); or one to three months related experience and/or training. Valid New York State Driver's License required and Valid First Aid Certification necessary.
Program Aides - Per Diem (Hauppauge)
Assists in implementation and recording of program services for individuals as outlined in each individual’s plan of care. Responsible for the protective oversight and safeguards of the individuals assigned throughout the program day. High school diploma or general education degree (GED) required. SCIP-R, AMAP and CPR certification preferred.
ADULT DAY SERVICES
Physical Therapist – P/T (Commack)
Responsible for the evaluation and treatment of those students who have been referred by a physician, and assigned by the Director of Educational Services. The student referred may have difficulties of orthopedic, neuro-muscular, and/or developmental origin. Bachelor's degree (B. A.) from four-year college or university. Graduate of a school accredited by APTA. Two years pediatric/rehabilitation experience and NYS License or eligibility upon graduation in Physical Therapy required.
Teacher of the Visually Impaired – F/T or P/T (Commack)
Provide appropriate individualized instruction to meet the needs of students according to their Individualized Family Service Plans and Individualized Education Plan. Minimum, Bachelor's degree (B. A.), preferably a Master of Science degree from a four-year college or university; some experience preferable. NYS Certification as a Teacher of the Visually Impaired required.
Teacher Aides Subs (Commack)
Assists classroom teacher in curriculum implementation by working with individuals or groups of children as assigned by classroom teacher. Assists in implementing strategies suggested by clinicians to the teacher. Performs frequent daily lifting of individuals with disabilities (ages 2-21), for their activities of daily living needs. High School Diploma or general education degree (GED) required.
Residence Managers (Various locations)
Manages one subordinate supervisor (Assistant Residence Manager) who supervises a total of up to 13 employees in the Residence. Responsible for the overall direction, coordination, and evaluation of these units. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. High school diploma or general education degree (GED) and one to three years related experience and/or training. Supervisory experience in the field of developmental disabilities necessary. Valid clean NYS Driver's License required. SCIP-R, CPR, and AMAP certification preferred.
Assistant Residence Managers (Various locations)
Assist Residence Manager to perform all duties related to oversight of assigned residential program serving individuals with developmental disabilities. Function as Manager in Manager's absence. Supervises a total of up to 13 employees in the Residence. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. High school diploma or general education degree (GED) and one to six months related experience and/or training. Supervisory experience in the field of developmental disabilities necessary. Valid clean NYS Driver's License required. SCIP-R, CPR, and AMAP certification preferred.
Social Worker - F/T (Commack)
Advocate for all participants of ICF programs and act as a liaison with state and federal agencies, parents, guardians, day treatment and habilitation programs. MSW and NYS License required.
Registered Nurses (P/T)
Responsible for monitoring and assisting in the overall health and safety of the individuals served. Insure that all indicated medical appointments are scheduled and followed up appropriately. Insure maintenance of all medical records per state and federal guidelines. Attend annual/interim meetings, weekly IDT meetings, and staff meetings. Participate in supervision and in-service training of direct care staff. Meet regularly with residence managers to discuss current health care concerns (minimum once weekly). Associate's degree (A. A.) or equivalent from two-year college or technical school and one to six months' related experience and/or training; or equivalent combination of education and experience. New York State - Licensed Registered Nurse required.
Confidential Secretary F/T (Commack)
Perform a variety of clerical functions related to the operation of Residential Programs. To perform this job successfully, an individual should have basic to intermediate knowledge of Microsoft Office software (Word, Outlook, Excel). Candidates must possess organizational skills and knowledge of basic office procedures. Ability to use office machines - computer and fax. Must pass Agency typing test (40 wpm). High school diploma or general education degree (GED) and one to six months related experience and/or training.
Direct Support Professionals- F/T, P/T & Per Diem (Various Locations)
Assist individuals with developmental disabilities in activities of daily living. High school degree or GED required. Must have a clean and valid NYS Driver’s License.
Physical Therapist F/T (Central Islip)
Responsible for the evaluation of patients referred for Physical therapy services and the formulation of a treatment care plan based on best current practices. Treatment of patients through the use of physical agents, appropriate therapeutic exercise, stretching, joint mobilization or other therapeutic technique and re-evaluation of patients on program. Bachelor's degree (B. A.) from four-year college or university and one to three years of experience (OMRDD population preferred). New York State License in Physical Therapy NYS Licensed Physical Therapist required.
Custodian – P/T (Commack)
Maintains building, performing minor and routine painting, plumbing, and other related maintenance activities. Insure that facility is free of unsafe conditions and conform to agency standards, state and federal regulations. High school diploma or general education degree (GED) and one to three months related experience and/or training. Must have a minimum one-year's experience in housekeeping or similar field and valid current New York State Driver License.
