ADULT DAY SERVICES

Day Habilitation Asst. - Long Term Temp. (Bohemia)
Responsible for the implementation of individualized services in all Day Habilitation areas emphasizing community integration and in-house activities to enhance the People Served in their chosen outcomes affecting his/her capacity to live, work, socialize, and participate within the community. High school diploma or general education degree (GED); or one to three months related experience and/or training. Valid clean New York State Driver's License.

Service Coordination Mgr. - (Marcus)
Oversee day-to-day operations of the Service Coordination Department. Train and supervise all Service Coordination staff in the delivery of case management services. Includes review of all ISP's. Responsible for implementation of all required regulations and documentation, including but not limited to CARF, NYOPDD, DOH and all other Federal, State and local regulatory agencies. Participate and represent the Agency at community meetings and presentations for customer based marketing. Master's degree (M.A.) or equivalent and a minimum of 5 years supervisory experience or equivalent combination. Experience with NYOPDD service coordination delivery a must. Clean and valid NYS driver's license required.

Confidential Health Office Asst. - P/T (Marcus)
Provide clerical and administrative support to the Adult Day Services Nursing Department. Handle paperwork, typing reports, answer phones and assist staff as necessary. Approximately 20 hours per week from 10a-2p Monday through Friday. Must have High School Diploma or GED.


RESIDENTIAL

Assistant Residence Managers (Various Locations)
Assist Residence Manager to perform all duties related to oversight of assigned residential program serving individuals with developmental disabilities. Function as Manager in Manager's absence. High school diploma or general education degree (GED) and one to six months related experience and/or training. Supervisory experience in the field of developmental disabilities Valid clean NYS Driver's License. SCIP-R, CPR, and AMAP certification preferred.

Direct Support Professionals (F/T & Per Diem) (Various locations)
Assist individuals with developmental disabilities in activities of daily living. High school degree or GED required. Must have a clean and valid drivers license.

Occupational Therapist (P/T) (Various Locations)
Bachelors Degree required. Graduate of school accredited by A.O.T.A. Must have valid NYS Drivers License.

Applied Behavioral Specialist - F/T & P/T (Various Locations)
Develop and maintain goal plans related specifically towards addressing behaviors of individuals served under to supervision of a licensed Psychologist. Responsible for evaluation and testing individuals served and training of staff members. Master's degree (M.A.) or equivalent; or four to ten years related experience and/or training; or equivalent combination of education and experience. M.A. degree in Applied Psychology, from an accredited training program (including internship) e.g., school, clinical, counseling, and educational.

Recreation Therapist - F/T (Various Locations
) Responsible to structure, incorporate and monitor meaningful individualized group activities for people with developmental disabilities. Bachelor's degree (B. A.) from four-year college or university and one to six months related experience. Certified Recreation Therapist, preferred.

RNs & LPNs (Full-time and Per Diem Available)


EDUCATION

Teacher Aides Subs (Commack)
Assists classroom teacher in curriculum implementation by working with individuals or groups of children as assigned by classroom teacher. Assists in implementing strategies suggested by clinicians to the teacher. Performs frequent daily lifting of individuals with disabilities (ages 2-21), for their activities of daily living needs. High school diploma or general education degree (GED) required.
